Maximus Decimus Meridius (his full name is stated only once in the film) is a fictitious character! There was in fact a general by the name of Avidius Cassius, who was involved in the military campaign shown in the film, and, upon hearing a rumor of Marcus Aurelius ‘ death, declared himself emperor.
Acknowledging that Maximus’s skill exceeds his own, Commodus deliberately stabs Maximus with a stiletto, puncturing his lung, and has the wound concealed beneath the gladiator’s armor. In the arena, the two exchange blows before Maximus rips the sword from Commodus’ hands.

Shortly before the film’s final sequence—a battle to the death between Commodus and Maximus in the arena—Commodus visits Maximus beneath the Colosseum. He stabs him in the back (yes, literally) and then orders Quintus to cover up the wound.
Maximus had been labelled a traitor to the empire: now led by Commodus, after the death of Marcus Aurelius. Maximus had been tasked by Marcus Aurelius to reform the Roman state as a republic, handing rule over to the people; this removed Commodus’ birthright, hence his slating Maximus for execution.
The film is loosely based on real events that occurred within the Roman Empire in the latter half of the 2nd century AD. As Ridley Scott wanted to portray Roman culture more accurately than in any previous film, he hired several historians as advisors.
